It's called the "Sturmtiger". There is a paper model for sale by WAK. I have no idea how you could get it, but you could fabricate one off of an early Tiger Chassis. There are 1000's of pictures of this on Google images, it shouldn't be hard to fabricate at all. It was a Mortar Tank, fired shell very high up, and came straight down. 
You can try http://www.minimodel.cz/ there are a lot of models you could use and many are possible with a re-color or new paint. Tons of buildings can be found at papermau.blogspot.com, or www.papercraftsquare.com that you can use as is or with some adjustment in the print scale...you can also do a search on pinterest www.pinterest.com/pin/188166090655300085/ and find many more !!!!
There are some GREAT tanks at http://i-cardtanks.com/ ...they are at 1/48th scale but a re-scale would work...they are HIGHLY detailed!!! and FREE...
